CIVIC IS CURRENTLY OPEN TO NEW ENROLMENTS - WITH RESTRICTIONS.
HOWEVER, WE WILL BE LIMITING OUR WEEKLY INTAKE AND ENROL new PATIENTS AS CAPACITY ALLOWS.
patients can continue to contact us and add their names to our waiting list and we will BE IN TOUCH when our capcity allows for any new enrolments.
patients who cAN enrol WITH THE PRACTICE INCLUDES those NEW TO THE MARLBOROUGH REGION WHO ARE currently UNENROLLED WITH A LOCAL PRACTICE AS WELL AS PATIENTS WHO WISH TO ENROL WITH civic FROM ANOTHER GP PRACTICE WITHIN MARLBOROUGH.
When we enroll new patients, our priority will remain with those who are new to the region and who are not yet registered with a local GP. Please contact our Administration team on 03 578 0199 for more information and how to provide your details for our waiting list.
Our enrolment process
New patients will be able to enrol by filling in an enrolment form and presenting it to our reception staff with valid form(s) of ID. When transferring from another medical centre (outside of the Marlborough region) we will advise them to transfer your medical records over to us. When you enrol, you may need to have a double GP appointment to ensure we obtain your health history and discuss your general health needs. The first GP appointment will therefore be charged double.
PLEASE BRING WITH YOU WHEN YOU ENROL
ONE valid form of PHOTO Identification such as Driver’s License, NZ or Overseas Passport, Kiwi Access Card etc.
Proof of Eligibility for Public Funded Health Services such as 2+ years’ Work Visa, Australian citizen/perm resident etc.
