ENROLMENTS

We regret to advise that we are currently NOT OPEN FOR ENROLMENTS.

Taking on new patients is a priority for us but At THIS TIME WE are working on ways to meet the demand. Please keep an eye on our website for updates or contact our support team at reception@civichealth.co.nz to discuss this.

When we reopen enrolments, new patients will be able to enrol by filling in an enrolment form and presenting it to our reception staff with TWO forms of ID. If you are transferring from another medical centre we will advise them to transfer your medical records over to us. When you enrol you may need to have a double GP appointment to obtain your health history and discuss your general health needs.

PLEASE BRING WITH YOU WHEN YOU ENROL

• ONE photocopy of your passport with any relevant visas OR a New Zealand birth certificate.

• ONE further form of identification, ONE needs to have a photo I.D (e.g drivers licence). ONE proof of address (e.g. bank statement, telephone bill)
