ENROLMENTS

We are currently taking on a limited number of new patients a week - please contact our support team - admin@civichealth.co.nz or click on the link to download the application form. We would be glad to hear from you.

New patients can enrol by filling in an enrolment form (contact us by email or collect at reception) and present it to our reception staff with TWO forms of ID. When you enrol you will need to have a nurse appointment before your doctor’s appointment to obtain your health history and discuss your general health needs. If you are transferring from another medical centre we will advise them to transfer your medical records over to us.

PLEASE BRING WITH YOU WHEN ENROLLING

• ONE Photocopy of your passport with any relevant visas OR a New Zealand birth certificate.

• ONE further forms of identification, ONE needs to have a photo I.D (e.g drivers license). ONE proof of address (e.g. bank statement, telephone bill)
